New Patient Registration

Please check at reception to confirm that you live within our practice area as we cover approximately 1 mile to the South East and West and 1 1/2 miles to the North. You can also check online in the 'Practice Registration Boundary' tab above this page.

We will require your NHS number when you register with us.  If you do not have your NHS Number, we need to ask for further details to enable our local Clinical Commissioning Group (CCG) to issue an NHS Number for you. Our reception team will be able to give you appropriate advice.

Your registration pack will include a GMS1 form, health questionnaire, opt-out form and information sheet, online access application form and a surgery leaflet We ask that you complete the forms as fully as possible before you come for your first appointment. To complete the new patient health check you can either make an appointment with our Health Care Assistant or come in during opening hours to use our Surgery Pod. A new patient health check is part of the required registration procedure.

You may also be invited for regular health checks in accordance with NHS policy. If you are over 75, you have the right to be offered an annual health check and will be told who your named GP is.
