Adelaide Medical Centre has been caring for the local community of Primrose Hill, Swiss Cottage, Chalk Farm, Belsize Park, Hampstead and Camden since 1974. We look after over 12,000 patients, with our dedicated team of 10+ experienced doctors and other clinicians including nurses, advanced physician assistants, pharmacists and social prescribers. We strive to deliver, in partnership with our patients, the highest standard of patient-centered care within the National Health Service. Practice Based Pharmacist

works as part of the general practice team to improve value and outcomes from medicines and will consult with patients directly. This includes providing extra help to conduct clinical medicines reviews, manage long-term conditions like hypertension, delivering advice for those on multiple medicines (polypharmacy), medicines safety audits & education and better access to health checks. They also assist with communication across a patient’s care pathway, manage medicines shortages by suggesting suitable alternatives where appropriate, and mentor newer pharmacists.
